A wound center receives a shipment of dehydrated amniotic membrane allografts. The box includes a Certificate of Analysis for each lot, listing sterility status, endotoxin levels, and a donor-eligibility reference. The procurement team files the certificates and the product moves to the shelf. But a Certificate of Analysis is a manufacturer's self-attestation — it documents what the producer measured and reported. The question that procurement committees and compliance officers are increasingly asking is how to independently verify what is actually in the box.
That question is the foundation of quality assurance for wound biologics. Tissue-based products — amniotic membrane, dehydrated chorion, cellular and tissue-based skin substitutes — enter the supply chain through manufacturers, distributors, and group purchasing organizations. Each handoff is a point where documentation can be complete but independent verification absent. This article outlines the testing standards, documentation requirements, and vendor-qualification practices that procurement teams can use to build independent verification into their biologics supply chain.
The Regulatory Framework
Human cells, tissues, and cellular and tissue-based products (HCT/Ps) are regulated under 21 CFR Part 1271. The regulation establishes two tiers based on intended use and manufacturing characteristics. Products meeting all four criteria in 21 CFR 1271.10(a) — commonly called "361 products" — are regulated solely under Section 361 of the Public Health Service Act and do not require pre-market approval. Products that do not meet all four criteria are regulated as biologics or devices under the 351 pathway or applicable device authorities, requiring pre-market review.
For all HCT/Ps, the regulation requires donor screening and testing for communicable diseases (21 CFR 1271.90), current Good Tissue Practice procedures to prevent disease transmission (21 CFR 1271.145), records maintenance (21 CFR 1271.210), and tracking from donor to recipient or final disposition (21 CFR 1271.220). These requirements form the regulatory floor for U.S. tissue establishments and establish the documentation trail that procurement teams should expect to verify.
Testing Standards That Matter
Independent verification of a tissue product's quality rests on a defined set of pharmacopeial tests. Each addresses a specific risk in tissue processing and handling.
Sterility. USP General Chapter <71> establishes the compendial method for sterility testing. A Certificate of Analysis should state that the product was tested per USP <71> and specify the result as a quantitative or qualitative finding — not merely a pass/fail checkmark. For aseptically processed products, both the processing method and the testing method should be documented.
Bacterial endotoxin. USP General Chapter <85> defines the Bacterial Endotoxins Test (BET), which quantifies endotoxin levels using Limulus amebocyte lysate (LAL)-based methods. Endotoxin contamination, even at levels below the threshold for systemic reaction, can provoke inflammatory responses in the wound bed that interfere with graft integration. The CoA should report endotoxin levels in endotoxin units (EU) and reference the applicable product-specific limit.
Identity and potency. USP General Chapter <1046> addresses cell-based therapies and outlines expectations for identity testing — confirming that the product contains the tissue type and biological components the label claims. For amniotic membrane products, identity testing may include confirmation of tissue origin and the presence of key structural proteins or growth factors. Potency — the measurable biological activity relevant to the product's intended effect — is more complex for tissue products than for synthesized pharmaceuticals, and the specific methods vary by product category. A CoA that lists only physical characteristics such as dimensions and thickness, without biological or identity data, warrants further questions.
Viability (for live-cell products). Cryopreserved or fresh products that contain living cells require viability testing to confirm that cells survived processing and storage. Dehydrated products do not contain living cells, and viability testing does not apply — but this distinction should be explicitly documented on the CoA, not assumed by the reader.
Reading a Certificate of Analysis
A Certificate of Analysis should contain, at minimum: the product name and lot identifier; the manufacturer name and establishment address; donor-eligibility status with reference to 21 CFR 1271 requirements; testing methods and quantitative results for sterility, endotoxin, and identity; storage and handling conditions; and the signature or controlled identifier of the responsible quality unit with an approval date.
Red flags include certificates that list "passed" or "compliant" without specifying the test method or numeric result; missing or unverifiable lot numbers; donor-eligibility references that cannot be traced to source documentation; CoAs that do not reference USP compendial chapters by number; and certificates lacking a quality-unit signature, approval date, or defined release criteria.
Chain-of-Custody Documentation
Tissue product tracking under 21 CFR 1271.220 requires that each establishment in the distribution chain — manufacturer, distributor, and consignee — maintain records linking the product from donor to final disposition. For procurement teams, the practical requirement is a documented chain of custody: records showing when the product left the manufacturer, which parties handled it at each step, and the temperature conditions maintained during transit.
Cold-chain integrity is particularly relevant for cryopreserved or fresh products. A temperature excursion during shipping can compromise product viability without any visible change to the packaging. Temperature logs or continuous electronic monitoring data should be part of the documentation received with each shipment and reviewed against the manufacturer's stated storage requirements.
AATB Accreditation as a Minimum Standard
The American Association of Tissue Banks (AATB) provides accreditation for tissue banks based on its Standards for Tissue Banking, which address donor screening, recovery, processing, storage, and distribution in greater specificity than the FDA regulatory floor. AATB accreditation requires periodic inspection, documented quality systems, and adherence to defined processing and testing standards. Procurement teams can use AATB accreditation status as a minimum qualification threshold for tissue suppliers. Status is verifiable through the AATB's published directory of accredited tissue banks.
Building a Vendor-Qualification Process with Independent Verification
A robust vendor-qualification process for wound biologics should include:
- Regulatory document review. Collect and review CoAs, donor-eligibility documentation, and FDA tissue establishment registrations (21 CFR 1271.22) for each supplier before adding the product to formulary.
- Accreditation verification. Confirm AATB accreditation status and review the scope of accreditation to ensure it covers the product categories being purchased.
- Independent lot testing. For high-volume or high-risk products, consider periodic independent third-party testing of received lots to verify sterility, endotoxin, and identity claims against the manufacturer's Certificate of Analysis. Independent testing labs serving this function are a maturing component of the biologics quality-assurance infrastructure.
- Inspection and enforcement review. Review FDA inspection findings — Form 483 observation reports and facility inspection classifications — available through FDA's public databases, as well as any warning letters relevant to tissue product manufacturers in the supply chain.
- Chain-of-custody standards. Establish minimum documentation requirements for all shipments, including temperature monitoring records for cold-chain products and signed receipt verification at each transfer point.
FDA Enforcement as Context
FDA maintains a public database of warning letters issued to tissue product manufacturers for deviations from current Good Tissue Practice and 21 CFR Part 1271 requirements. These enforcement actions — which have included citations for inadequate donor screening, insufficient sterility testing, and incomplete records — are publicly accessible and illustrate why independent verification is a prudent component of procurement practice. Specific manufacturers and products are not identified in this article; the relevant point for procurement teams is that regulatory compliance cannot be assumed from the existence of a Certificate of Analysis alone.
